Initial Comments:
An off-site follow-up survey conducted on July 11, 2022 found that C &; OPD, LLC had corrected the deficiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a State relicensure and Medicare recertification survey completed on April 15, 2022.
